During your initial consultation, I will complete a comprehensive health assessment and medical history. It may be important in your prognosis to review and interpret your lab work, particularly if you have a chronic illness.
If you are a first-time patient, please download the patient forms, fill them out, and bring them with you or send electronically (for phone consultations). If you have any lab tests, please bring them with you or send them in advance.
These consultations will be a continuum of your treatment plan and modified according to your progress and changes within your own body. Options (including package pricing) are available to promote your successful journey to vibrant health.
